Our Services: Special Services Authorization
If a special service is requested or an actual number of visits per week would be greater than allowed for the intermittent nursing visit, a service authorization form must be filled out prior to the rendering of the service. If you are a Medicaid recipient, a service authorization would be obtained prior to the administration of service. Authorization would also be obtained for skilled nursing services that would be provided in excess of 30 days. Back to our services directory
