Our Services: How to Sign Up with RMG Health

From the Hospital:

Call RMG Health LLC and request for a "Home Care Planner". The planner will discuss and evaluate eligibility for home health or hospice services with the client hospital physician. The planner will make the official request for a home care assessment and evaluation. Planner visits are free.

From a Nursing Home:

Your physician can order RMG Health service through the nursing home-care or social service department of the nursing home agency. Or better, you can call the RMG Health "Home Care Planner" directly and request a visit.

From Home:

Call RMG Health and request for the Intake Coordinator to discuss the need and eligibility for RMG Health services. Family members can also call on client behalf. RMG Health will get in touch with client physician to make a referral for a home evaluation and initiate the intake protocol. Back to our services directory
